General Information
What is a score verification session? This is a two-and-a-half hour session to review possible scoring errors. It is held in Tallahassee eight times a year on Saturdays.
Who is qualified to attend? Examinees who have taken and NOT PASSED one or more paper-and-pencil or computer-based tests are qualified to review their test(s) for possible scoring errors if they submit the printed Score Verification Request Form included with their official score reports within 30 calendar days after the score report was mailed to them.
What happens at the session? At the session, you are given the opportunity to review the test(s) you took and appeal any item(s) you think were scored incorrectly. You will be given a test book that corresponds to the test you took and a printout that lists only your incorrect answers.
If you wish to appeal a test item, you may ask for a Challenge form and submit it before the end of the session. This form, if completed correctly, will be reviewed by the Department of Education and a response sent to you within 30 calendar days after the session.
Is there a fee for registering for a score verification session? There is a nonrefundable processing fee of $25 in the form of a money order or cashier’s check made payable to CALA/FSU.
IMPORTANT: If you are scheduled for a session and fail to attend that session because of an illness or death in the immediate family, and you can provide proper documentation of the reason, your registration form and fee will be transferred to the next session. Your money WILL NOT be refunded.
How do I register? You must wait until you receive your score report before submitting a request for a score verification session. To register for the score verification session, fill out the Score Verification Request Form you received with your score report from National Evaluation Systems and mail it with your fee payment to the CALA office at Florida State University in Tallahassee before the 30-calendar-day deadline. This address is listed on the Score Verification Request Form. You must register using this form because it is specifically dated for the score verification session for which you are eligible. You may NOT KEEP this form or copy it to use for score verification sessions not listed on your form.
Score Verification Request Forms sent without the processing fee WILL NOT be accepted and will be returned. If you submit a request form with inaccurate information that results in the invalidation of your request form, the processing fee WILL NOT be refunded. Causes for invalidation include, but are not limited to, the use of an incorrect test name or test date, or the submission of an incorrectly completed or out-of-date Score Verification Request Form.
DO NOT SEND CASH OR PERSONAL CHECKS!

Is transfer of registration allowable? Transfer of registration is only allowable when the registered examinee fails to attend a scheduled session due to
- personal illness and the examinee can provide appropriate documentation, or
- illness or death in the immediate family and the examinee can provide appropriate documentation, or
- in case of a natural disaster or local emergency.
All transfer requests must be approved by the Florida Department of Education and CALA/FSU.
Examinees who have been granted a transfer and fail to attend the next session will forfeit their right to review the requested test material.
What if I need special accommodations? If you need special accommodations because of a disability or a religious exception to a Saturday session, you must complete the “Special Accommodations” section on the Score Verification Request Form. Your request will be accepted if you were granted this accommodation at the time you took the exam. If you did NOT RECEIVE special accommodations at the time of the exam, you must meet the criteria established by the Florida Department of Education.
